Introducing the Collaborative Health Record
Patient health records within most health systems are dangerously fragmented — forcing providers to make diagnoses and recommendations with limited patient information at the point of care. At the most recent approximation, only 10 percent of ambulatory practices and 30 percent of hospitals were significantly participating in operational health information exchange efforts.
In an effort to empower physicians with the ability to make more informed clinical decisions, and in-turn catalyze a more holistic, responsive, and patient-centric network of health care delivery, we’re excited to introduce the Collaborative Health Record (CHR).
The CHR embodies Elation’s efforts toward developing a patient record that allows all providers caring for a mutual patient to easily share patient data and coordinate care in real time. By combining the benefits of a Health Information Exchange with the functionality of a Clinical First EHR, this powerful clinical tool gives providers a more complete longitudinal picture of a patient’s health without leaving the exam room.
This innovative new approach to designing and using patient records represents the evolution of the Electronic Health Record (EHR) to a Collaborative Health Record (CHR). Instead of simply digitizing paper records, the CHR is a powerful tool that’s built around patient data in order to foster a more collaborative care environment.
By digitally linking an entire patient’s care network, the CHR helps physicians seamlessly update their colleagues about new patient information, helping the entire Care Team to provide the highest quality care for patients — across systems and between providers.
The CHR works through the Collaborative View tool, which helps multiple providers caring for the same patient to automatically share patient information and coordinate care. Providers have access to a comprehensive view of each provider’s shared activity. As a result, this complete longitudinal history includes the most up-to-date information on a patient’s personal chart and can be easily shared and used at the point of care.
With the Collaborative View, providers are able to:
