Elation is now PCMH Pre-Validated

The patient-centered medical home (PCMH) structure enables the primary care physician to provide continuous and coordinated care to patients from a home base. All of the patient’s healthcare needs, including preventative services, diagnosis and treatment of illnesses, and even end-of-life care, are arranged within the PCMH. Elation Health dynamically supports the concept of the PCMH and we are now proud to announce that we are PCMH Pre-Validated, a valuable recognition earned from the National Committee for Quality Assurance (NCQA).

NCQA is a private, nonprofit organization dedicated to improving health care quality. NCQA accredits and certifies a wide range of health care organizations. It also recognizes clinicians and practices in key areas of performance. NCQA Patient-Centered Medical Home (PCMH) Recognition is the most widely used PCMH evaluation program in the U.S.

The Agency for Healthcare Research and Quality (AHRQ) defines a medical home “not simply as a place but as a model of the organization of primary care that delivers the core functions of primary health care.” PCMH functions and attributes include:
